The following information is required for each speaker for Continuing Education Credit submissions and promotion purposes. Please email your head-shot, resume and presentation summary to assistant@nvhca.org.

19 Speaker Registration

  • Date Format: MM slash DD slash YYYY
  • Please list all of your education with School, Degree & Grad date
  • NVHCA will supply a handheld and/or a lavaliere microphone, computer system with PowerPoint software, projection and screen configuration. Let us know if you need anything outside of these items for your presentation.
  • Enter the dollar value for the presentation fee
  • Enter the dollar value for the travel reimbursement amount
  • Enter mm/dd/yyyy
